Ballot Issue Crisis Fund 
I understand that my annual contribution to the Ballot Issue Crisis Fund is not new and has been collected as part of my dues for the 2017-18 membership year. In signing, I authorize continuing my current voluntary contribution of $13 (ActiveProfessional) or $6 (Active Support Professional). I can adjust that rate up or down by writing a different amount here

Local and State Legislative Political Action Fund
In signing, I authorize an annual $12 voluntary contribution--$1 a month--(split 50/50 between my local and state legislative/political action fund). The local fund is used to support local levy/bond elections or to support recommended school board candidates. Or I choose to adjust the amount to * $ .00
